n • Monday–Friday | 8:00 AM–5:00 PMn
n • Shared call rotationn
n • In-person and virtual visits supported by Medical Assistantn
n • Manage complex patients with ongoing follow-up throughout stayn
n • Collaborate with SNF team to facilitate care coordinationn
n • Implement discharge planning to improve patient outcomesn
n • Signing bonus, CME, and relocation available.n
