Speech Language Pathologist
Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in skilled nursing facility residents. Develop and implement individualized treatment plans to promote independence, safety, and quality of life. Provide direct therapy services targeting speech, language, cognition, and swallowing functions. Perform and interpret swallowing assessments and recommend diet modifications. Document evaluations, treatments, progress, and discharge summaries in compliance with regulations. Educate residents, families, and staff on therapy plans, communication strategies, and safe swallowing techniques. Collaborate with interdisciplinary care team and participate in care planning meetings. Monitor resident progress and adjust treatment plans as needed. Ensure compliance with professional, legal, and regulatory standards. Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs), if applicable.
