Position Overview

Shift Details
Shifts will be during the day, with a schedule of 5 days a week, working 8 hours per day from 08:00 AM to 05:00 PM.
Key Responsibilities
- Conduct home health visits with a minimum of 6 points per day.
- Manage dysphasia and implement aural rehabilitation programs.
- Conduct evaluations using tools such as BIMS and depression screenings.
- Provide patient and family education on care plans and rehabilitation strategies.
- Maintain accurate billing and documentation per regulations.
- Complete functional maintenance programs.
- Participate in in-service training sessions.
- Minimum of 1 year home health experience required.
- OASIS training is preferred.
- Valid BLS and CCC-SLP certifications required.
- Current state licensure as applicable.
- Experience with electronic health record systems, preferably Cerner.
- Must have a valid driver's license and access to a vehicle for home visits.
- Candidates must be COVID-19 vaccinated or provide an exemption form.
- Flu shot is required unless a declination is provided; mask-wearing is mandatory.
