Service Facilitator

Minimum of 30 hours per week.
Bachelor's or Master's degree in Social Work, Psychology, or a related human services field.
At least two years of relevant experience, including experience providing case management services.
The position includes remote work and direct work with clients in the community.

Key Responsibilities
Develop and maintain collaborative relationships with clients, families, natural supports, and service providers.
Coordinate the client's individualized services and ensure supports remain connected to the client's identified recovery goals and needs.
Participate in screening, assessment, service planning, recovery planning, and ongoing review of client progress.
Develop, update, and monitor individualized recovery plans and action steps with measurable, client-centered goals.
Help clients identify needed services, providers, community resources, and natural supports and assist with accessing those resources.
Maintain regular contact with clients and providers to monitor service delivery, engagement, progress, barriers, and changing needs.
Facilitate communication among members of the client's recovery team and support coordinated, person-centered service delivery.
Advocate for client choice, preferences, rights, cultural needs, and participation in all aspects of service planning.
Identify barriers to progress and work collaboratively with the client and service team to address them.
Monitor authorizations, service needs, plan review dates, and required follow-up to help ensure continuity of care.
Complete accurate and timely documentation of contacts, coordination activities, service planning, and client progress.
Respond appropriately to changes in client needs, including safety concerns or crises, and coordinate additional supports when necessary.
