Seeking a compassionate Service Facilitator to support individuals and families through the Comprehensive Community Services (CCS) program.
Responsibilities Include
- Conducting functional and needs assessments.
- Developing and implementing person-centered service plans.
- Coordinating services and connecting clients with community resources.
- Facilitating team meetings and collaborating with families, providers, and support networks.
- Maintaining accurate documentation and client records.
- Advocating for clients using trauma-informed, strengths-based practices.
Qualifications And Skills
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
- Experience with vulnerable populations preferred.
- Strong communication, organization, and relationship-building skills.
- Ability to manage documentation, coordinate multiple services, and work independently.
- Valid driver's license, reliable transportation, and ability to travel within the community.
