Service Facilitator (Full-Time)

Service Facilitator role involves partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate support services that foster independence and well-being.

Responsibilities include conducting functional assessments, creating person-centered plans, connecting clients with community resources, facilitating team meetings, maintaining accurate documentation, and advocating for clients using trauma-informed, strengths-based practices.

Qualifications required are a bachelor's degree in Social Work, Psychology, Human Services, or related fields, with preferred experience working with vulnerable populations. Strong communication, organizational, and relationship-building skills are essential, along with the ability to manage multiple services and work independently. A valid driver’s license and reliable transportation are needed for community travel.
