Seeking a compassionate Service Facilitator for the CCS Program to support individuals and families in accessing community resources and developing personalized service plans.
Responsibilities Include
- Conducting functional and needs assessments
- Developing and implementing person-centered plans
- Coordinating services and connecting clients to community resources
- Facilitating team meetings and collaborating with families, providers, and support networks
- Maintaining accurate documentation and client records
- Advocating for clients using trauma-informed, strengths-based approaches
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
- Experience with vulnerable populations preferred
- Strong communication, organization, and relationship-building skills
- Ability to manage documentation, coordinate multiple services, work independently
- Valid driver's license, reliable transportation, and ability to travel within the community
