Service Facilitator (Full-Time)

We are seeking a compassionate Service Facilitator to support individuals and families through the Comprehensive Community Services (CCS) program.

Key responsibilities include conducting needs assessments, developing personalized service plans, and coordinating services with community resources. The role involves facilitating team meetings, collaborating with families and providers, maintaining accurate documentation, and advocating for clients using trauma-informed, strengths-based practices.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
• Preferred experience working with vulnerable populations.
• Strong communication, organization, and relationship-building skills.
• Ability to manage documentation, coordinate multiple services, and work independently.
• Valid driver's license, reliable transportation, and willingness to travel within the community.
