Service Facilitator (Full-Time)

Seeking a compassionate and organized Service Facilitator for the CCS program.

This role involves

• conducting functional and needs assessments,
• developing person-centered service plans,
• coordinating services and connecting clients with community resources,
• facilitating team meetings and collaborating with families and providers,
• maintaining accurate documentation,
• advocating for clients using trauma-informed, strengths-based approaches.

Qualifications Include

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
• Experience with individuals, families, or vulnerable populations preferred.
• Strong communication, organization, and relationship-building skills.
• Ability to manage documentation, coordinate services, and work independently.
• Valid driver's license, reliable transportation, and ability to travel within the community.
