Lutheran Social Services of WI & UP MI seeks a Service Facilitator to coordinate community-based services for individuals and families. In this role, you will assess client needs, develop person-centered service plans, and connect clients to behavioral health, housing, and supportive resources. You’ll collaborate with multidisciplinary teams, monitor progress, maintain accurate documentation, and ensure trauma‑informed, culturally responsive care.
