Role:
A Service Facilitator supports individuals with mental health challenges through care coordination, skill development, and treatment planning within a community-based setting.
Responsibilities:
Conduct assessments, develop and implement personalized service plans, facilitate recovery-focused team meetings, connect clients to community resources, and maintain accurate documentation. Collaborate with treatment teams, providers, and support systems to promote client independence and well-being.
Qualifications:
Master's degree in human services or related field, valid driver's license, ability to travel within the community, and flexible hours. Preferred experience in mental health, case management, or community services, especially with children and families. Relevant degrees include social work, psychology, counseling, and related fields.
