Service Facilitator (CCS)

We are seeking a compassionate Service Facilitator to join a community-based mental health team.

Key Responsibilities Include

• Conducting assessments and developing individualized service plans in collaboration with clients and support systems.
• Coordinating, authorizing, and facilitating recovery-focused, family-centered team meetings.
• Connecting clients to community resources and documenting all interactions accurately.
• Collaborating with treatment teams, community providers, schools, and natural supports to ensure comprehensive care.

Qualifications And Skills Required

• A master's degree in a human services-related field such as social work, psychology, counseling, or related disciplines.
• Valid driver's license and reliable transportation with ability to travel within the community.
• Ability to work flexible hours, including occasional evenings.
• Preferred experience in mental health, behavioral health, case management, or community-based services, especially with children, families, or individuals with complex needs.

• Compensation varies based on education and credentials, ranging from $27.20 to $30.20 per hour.
• Requires strong communication, organizational, and collaboration skills.
