Service Facilitator (CCS)

Seeking a compassionate Service Facilitator for the CCS Program, responsible for assessing client needs, developing personalized service plans, and coordinating community resources to promote independence and well-being.

Key Duties Include

• Conducting functional and needs assessments.
• Creating and implementing person-centered service plans.
• Connecting clients with community resources and services.
• Facilitating team meetings and collaborating with families, providers, and support networks.
• Maintaining accurate documentation and client records.
• Advocating for clients using trauma-informed and strengths-based approaches.

Qualifications And Skills

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
• Preferred experience working with individuals, families, or vulnerable populations.
• Strong communication, organization, and relationship-building skills.
• Ability to manage documentation, coordinate multiple services, and work independently.
• Valid driver's license, reliable transportation, and willingness to travel within the community.
