We are seeking a dedicated Service Facilitator to support individuals with mental health challenges through a person-centered, recovery-focused approach.
Key Responsibilities Include
- Conducting functional assessments and developing individualized service plans in collaboration with clients, families, and providers.
- Coordinating and authorizing community-based services and facilitating recovery-focused team meetings.
- Connecting clients to community resources and support systems.
- Maintaining accurate documentation, completing reports, and collaborating with treatment teams and community partners.
- Master's degree in a human services-related field such as Social Work, Psychology, Counseling, or related disciplines.
- Valid driver’s license and reliable transportation for community travel.
- Ability to work flexible hours, including occasional evenings, as needed.
- Providing mental health, behavioral health, therapy, case management, or care coordination services.
- Working with children, families, and adults with complex needs in recovery-oriented, trauma-informed, or person-centered settings.
