We are seeking a dedicated Service Facilitator to support individuals and families with mental health challenges through a person-centered, recovery-focused approach.
In this community-based role, you will
- conduct assessments and develop personalized service plans
- coordinate and authorize services
- facilitate team meetings
- connect clients to community resources
- maintain accurate documentation
- collaborate with treatment teams and community providers
Qualifications include a master's degree in a human services-related field and a valid driver’s license. Preferred experience involves mental health, behavioral health, case management, or community-based services, especially with diverse populations and trauma-informed care.
Key skills required are care coordination, excellent communication, flexibility, and the ability to work in various community settings. Knowledge of recovery-oriented, trauma-informed, and person-centered service delivery is beneficial.
