Join Mosaic to empower individuals, coordinate compassionate care, and build meaningful relationships!What You'll DoCoordinate & Support: Receive new referrals, manage intakes, and create personalized service plans for home and community-based services.Connect in the Field: Conduct regular home visits to ensure direct care quality and support standards are met.Build Relationships: Partner closely with clients, case managers, direct care workers, and community partners.Maintain Operations: Oversee scheduling adjustments, handle incident investigations, and provide rotating after-hours on-call support.What You BringEducation & Experience: High school diploma or equivalent required.Proven Track Record: Minimum 3 years in service coordination/delivery and 2 years in a supervisory role.Certifications: CPR/First Aid, Article 9, and Principles of Caregiver certification (or Instructor status).Core Skills: Strong organizational skills, computer proficiency, accurate record-keeping, and problem-solving ability.Language Skills: Spanish or Russian bilingual skills are highly desirable (preferred)!Ready to make an impact?
