EmpowerMe Wellness seeks therapists to provide physical, occupational, or speech therapy for older adults within our EmpowerMe 360 program. You will assess residents, build personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
You will also support care coordination through wellness check-ins, caregiver communication, and telehealth, while upholding professional ethics and contributing to program development to improve quality of care.
