You will manage a patient panel of ~500 Medicare Advantage patients, focusing on prevention, chronic disease management, and healthy aging. Expect 30-45 minute visits, 9:00 AM to 3:30 PM schedules, and a strong emphasis on outcomes, patient experience, and collaboration in a growing organization.
