Senior Nurse Care Manager — Hybrid, Lead Complex Care Plans

Blue Shield of CA is seeking a Care Management leader to independently manage high-risk cases using advanced clinical judgment. You will collaborate with providers, medical directors, and interdisciplinary teams to coordinate care, support discharge planning, and ensure referrals and continuity of treatment.

The role requires reassessment of care plans, knowledge of guidelines for chronic conditions, and participation in rotating evening/weekend on-call shifts.
