Greenbrook Medical, serving the Greater Tampa Bay Area, seeks a Complex Care Manager to coordinate care for the most medically complex patients from hospital to home. You will work with physicians, center managers, pharmacists, and hospital teams to ensure proactive, coordinated care across settings.
You will report to the VP, Market Medical Director and travel between clinics. A valid RN or LPN license, 3+ years in nursing/care management, and strong data skills are essential.
