Senior Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Senior Case Manager to coordinate complex patient care across our inpatient rehabilitation continuum. In this role, you’ll lead discharge planning, manage caseloads, conduct psychosocial assessments, and collaborate with an interdisciplinary team to support patients recovering from stroke, brain injury, spinal cord injury, and other conditions. You’ll advocate for patient needs, secure post-acute services, educate families, ensure regulatory and documentation compliance, and help drive quality, patient-centered outcomes in a mission-driven environment.
