You will lead a diverse caseload, collaborate with clinicians, and mentor junior staff to ensure continuity of care and optimal outcomes.
The ideal candidate has a Bachelor’s degree in a health-related field, at least 2 years of case management/home-based services experience, and CCM certification or plan to obtain CCM within a year.
