Description UCLA Health is seeking a dedicated Senior Care Physician to join our innovative Senior Care Model clinic in Encino. This role is designed to deliver high-value, patient-centered primary care for adults aged 65 and older. Built on the Age-Friendly Health System's "5Ms" framework-What Matters, Medication, Mentation, Mobility, and Multi-Complexity-this clinic will provide longitudinal primary and acute care services tailored to the needs of the aging population. - Provide comprehensive primary care for a smaller patient panel to enhance patient access and personalized care.
- Enjoy sharing quality visit times with your patients with longer visits
- Work in a collaborative, interdisciplinary team including Clinical Advisor RNs, Comprehensive Care Coordinators, and Licensed Clinical Social Workers to support patient care.
- Dedicate 32 hours per week to direct patient care, with additional time allocated for administration and chronic care management.
- Develop professionally through specialized education focused on chronic care management, value-based care, and population health strategies.
- Collaborate with the Senior Care Model Medical Director to drive quality improvement initiatives and evaluate new care delivery models.

- Board certified/eligible in Internal Medicine or Family Medicine, required.
- Board certified/eligible in Geriatric Medicine, preferred.
- California licensed or licensed-eligible, required.
- Passion for delivering patient-centered geriatric care and improving health outcomes for older adults.
- Ideal candidates will be comfortable with telehealth appointments, as needed.
