Senior Care Patient Navigator & Care Coordinator

Curana Health is seeking a Patient Navigator to serve as the first point of contact for patients, handling calls with professionalism and compassion. You will perform phone-based assessments, gather medical history, and guide patients to appropriate care while relaying information to healthcare teams to ensure coordinated, seamless care across the network.

The role requires a High School Diploma or equivalent, exceptional customer service, and strong communication and confidentiality skills.
