Responsibilities

n
• n

  Conduct student speech and language evaluations

  n
n
• n

  Develop and implement individualized treatment plans

  n

• n

  Provide direct speech-language therapy services

  n
n
• n

  Maintain thorough documentation in accordance with school and state requirements

  n

• n

  Collaborate with teachers, parents, and multidisciplinary teams

  n

• n

  Participate in IEP meetings and support student educational goals

  n
n

Minimum Requirements

• n

  Active Arizona Speech-Language Pathologist License

  n
n
• n

  Certificate of Clinical Competence (CCC-SLP) preferred and may be required

  n

• n

  Compliance with all professional standards of practice

  n
n
• n

  Current CPR certification (if applicable)

  n
n
• n

  Current TB screening and health certificate (if applicable)

  n
n
• n

  Must meet all federal, state, and local requirements

• n

  Must be at least 18 years of age

The Speech-Language Pathologist in the school setting is responsible for performing student evaluations as well as providing and documenting therapy services in accordance with the plan of care developed for each individual student and the physician's orders.

Minimum Requirements:

  Active Speech Language Pathologist (SLP) Licensure in the state of assignment required

  n

• n

  Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred and may be required by state/contract.

  Complies with all relevant professional standards of practice

  n

• n

  Current CPR if applicable

  n
n
• n

  TB questionnaire, PPD or chest x-ray if applicable

  n

• n

  Current Health certificate (per contract or state regulation)

  n

• n

  Must meet all federal, state and local requirements
