Salary starting at $50,000 - $1,000 Sign on Bonus! (excludes PRN Staff; Attendance and Performance considered)

South Texas Community Living is hiring a full-time or part-time LVN to provide nursing coordination to individuals with intellectual disabilities in group home and natural home settings. This position is similar to home-health nursing, with some duties and documentation specific to meet the unique requirements of HCS and/or ICF regulations. Our program provides a rewarding job experience for anyone interested in promoting the independence of people with disabilities.

FT Shift(s): Monday – Friday Hours: 11:00AM – 7:00PM or 7:00AM-3:00PM

LVN Duties include but are not limited to :

  • Patient assessment
  • Monitoring unlicensed personnel (direct care staff) in the performance tasks delegated by the supervising RN.
  • Training of direct care staff
  • Ordering medications and monitoring medication supplies
  • Coordinating medical appointments.
  • Communicating medical issues to family members of patients, and
  • General Nursing task as assigned.

Qualifications

Qualifications:

  • Valid LVN License Required
  • Valid and current Texas Driver's License
  • Clear Background Check
  • Clear DMV Check
  • High School Diploma or Equivalent Proof of Education
  • No confirmation of Abuse/Neglect/Exploitation of an elderly or disabled person
  • No history of Medicaid fraud.
  • Experience working with people with intellectual and developmental disabilities (group home experience preferred)
  • Strong communication and team-participation skills; friendly, positive attitude.
  • General computer skills (not necessarily expert); be able to use Word, Excel, Outlook, and general internet navigation.
  • Must have excellent time management skills.
  • Must be capable of multi-tasking in a fast-paced environment.
