VitalCaring is seeking a Sales Care Transition Navigator to connect patients, families, and providers with our home health and hospice services. In this field-based role, you will build strong relationships with hospitals, physicians, and post-acute partners, educating them on VitalCaring’s programs and coordinating smooth transitions of care to the home. You will meet with patients and caregivers to explain services, support informed decisions, and collaborate closely with clinical and intake teams to ensure timely, appropriate admissions.
