Salary: $5,000/month

About the role:

Cancer Rehab Group delivers oncology rehabilitation by video into patients homes. Our

physical therapists, speech-language pathologists, and cancer exercise physiologists handle the

therapy, and you handle everything else. You make sure referred patients get into care and stay

in it, which means owning every non-clinical step between a referral landing and a patient

finishing their program.


This pilot serves cancer patients and survivors in 3 rural southeastern Indiana counties, where

the nearest oncology rehab is a 2 to 3 hour round trip to Indianapolis. Over 10 months you

help screen about 130 people and enroll about 80. When a patient drops out because a tablet

won't connect, or because nobody called after a hospital stay, that's the gap this job exists to

close.


This is a non-clinical role. When a patient reports a new symptom or a change in how they're

functioning, you route it to their clinician the same day.


Core Responsibilities:

Get referrals in and confirm eligibility. Take referrals from partner oncology practices and

rural primary care offices. Confirm each patient's eligibility for the pilot, explain the program, and

get them scheduled.


Get patients connected before the first session. Walk each patient and caregiver through

setup on their own phone, tablet, or computer, plus the home kit we mail them. Run a

connection test before the first visit. If the connection won't hold video, move the patient to the

backup path so no one is turned away for lack of bandwidth.


Keep patients in care. Own the schedule. Troubleshoot connection problems before and

during sessions. When treatment or a hospitalization pauses someone's participation, their

sessions pause, and you reach back out on a set schedule to bring them back.

Collect outcomes by phone. Some patients can't finish the digital surveys at baseline,

midpoint, and completion. Call them and walk through the surveys by phone, then record the

responses in our clinical platform/EMR.


Close the loop on every referral out. When a screen shows a patient needs billable skilled

therapy, you make the referral and follow it until you've documented that the patient got care, or

why they didn't. When screening turns up a social need (transportation, food, utilities), you

connect the patient to Indiana 211.


Show up at referral sites. Visit partner practices in Decatur, Scott, and Franklin Counties each

quarter. Join the monthly check-ins with each referral partner.


Log what happens. Record connection type, session completion, and every drop to a backup

path, plus referral-loop status. The Data and Outcomes Coordinator builds the pilot's reports to

the state from what you log, so it needs to be complete.


Required Qualifications:

● Care coordination or community health worker experience; excellent communication skills, patient-centered and mission focused

● Comfort helping patients, including older adults with limited tech experience, get set up

on devices and video calls over the phone

● Valid driver's license and reliable transportation for quarterly site visits


Preferred Qualifications:

● Experience with patients in active cancer treatment or other serious illness

● Experience working in an EMR or patient engagement platform
