We're launching a Chronic Care Management (CCM) + Remote Patient Monitoring (RPM) practice inside our urgent care clinic and we're hiring ONE experienced program builder to stand it up end-to-end, from zero to a running, billing, growing program. This is not a staffing role. You'll own the build.
What you'll own (day one):
- Design and launch the full CCM/RPM program: patient identification, enrollment, consent workflows, device logistics
- CMS billing compliance and capture: RPM 99453/99454/99457/99458, CCM 99490/99491/99439
- RPM device selection, procurement, shipping, and troubleshooting logistics
- EHR + RPM platform integration and documentation workflows
- Hiring and training care coordinators; building the enrollment team
- Dashboards, KPIs, and monthly billing close grow census from 0 to 150+ patients
What we're looking for:
- 5+ years STANDING UP (not just working in) CCM and/or RPM programs vendor implementation teams, value-based care groups, MSOs, or large practices
- Deep, working knowledge of CMS CCM/RPM billing rules and documentation requirements
- Track record: programs launched, census grown from zero, billing-capture rates improved
- RN/BSN preferred (LPN/LVN or healthcare-admin background with equivalent ops experience considered)
- Device logistics + EHR fluency; bilingual English/Spanish a plus

1. Walk me through a CCM or RPM program you built from scratch what was the census when you started vs. when you left?
2. Which CMS billing codes did you own, and how did you make sure time and documentation supported every claim?
