Job Summary

Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.

Responsibilities

  • Conduct health assessments focused on wound, ostomy, and continence related care. Perform focused physical assessment, develop plan of care based on the diagnosis and individual client needs, and recommend treatments as appropriate.
  • Assist with development, revisions, and evaluations of standards of care.
  • Assess and document patient response to wound care, ostomy, continence, fistula, and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff.
  • Work collaboratively with the health care team and make referrals to other disciplines as indicated.
  • Teach, counsel, and assist individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems.
  • Provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures.
  • Participate in Unit/Department Quality Improvement and Hospital Wide Q.I. activities, e.g., Incidence, Prevalence Studies and Nursing Quality.
  • Provide culturally competent patient, family and staff education consistent with cultural, ethnic, religious and other belief systems.
  • Maintain the security and confidentiality of data and information in accordance with patient rights policies.
  • Perform other tasks as assigned.

Required Skills & Experience

  • Three to five (3-5) years of experience in healthcare (in addition to RN contracted labor requirements).
  • Ability to fully utilize facility electronic record systems, equipment and other healthcare and billing systems relevant to this position.
  • Good interpersonal skills.

Required Certifications & Licensure

  • Active NYS Registered Nurse license.
  • Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB;
  • Certified Wound Care Nurse (CWCN) issued by the WOCNCB;
  • Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM);
  • Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
