RN Triage & Patient Care Navigator

Community Action, Inc. is seeking an Registered Nurse – Triage, Primary Care in Ashwaubenon/Green Bay, WI. The role focuses on telephone triage, clinical guidance, and coordinating care within a busy primary care setting.

The RN will assess patient needs, provide safe, timely advice, and may deliver direct patient care. Wisconsin RN licensure and BLS/CPR are required upon hire, with a BSN preferred for this clinic position.
