CarePartners is seeking an RN Primary Case Manager to provide coordinated home health services to adult and geriatric patients across Connecticut. In this role, you will perform comprehensive assessments, develop and manage individualized care plans, deliver skilled nursing care, and collaborate with an interdisciplinary team. You will educate patients and families, monitor changes in condition, and ensure regulatory and documentation compliance.
