Home Health of Western Colorado seeks a compassionate RN to provide in-home skilled nursing care across Western Colorado. The RN conducts comprehensive assessments, administers treatments and medications, manages wounds, and coordinates care with an interdisciplinary team. Responsibilities include developing individualized care plans, educating patients and families, documenting in an EMR, and communicating with physicians. This patient-centered role offers meaningful one-on-one care, autonomy in the field, strong teamwork, and the opportunity to support patients in maintaining safety, independence, and quality of life at home.
