Registered Nurse (RN) – Per Diem NOC ShiftGarden City Healthcare CenterLocation: Modesto, CAPay Range: $40.00 – $45.00 per hour (DOE)Shift: Per Diem / OvernightShift Differential: Additional NOC differential providedStatus: Full-TimeSetting: Skilled Nursing Facility (SNF)General PurposeThe primary responsibility of the Registered Nurse (RN) is to supervise and provide direct nursing care during the assigned NOC shift in accordance with current federal, state, and local regulations. The RN ensures the highest degree of quality care is maintained and supports safe, effective overnight operations of the nursing department.Essential Duties and ResponsibilitiesAdministrative & Leadership FunctionsAssist the Director of Nursing (DON) and/or ADON with oversight of nursing services during the NOC shiftEnsure nursing staff comply with facility policies, procedures, and job descriptionsParticipate in quality assurance activities, audits, and facility surveys as requestedComplete required documentation, reports, and incident/accident reportsProtect resident confidentiality and report any unauthorized access to information systemsPersonnel & SupervisionEnsure adequate RN/LVN and CNA staffing levels throughout the shiftSupervise, support, and delegate duties to nursing staffConduct rounds to ensure quality care and compliance with nursing standardsPromote a calm, respectful, and resident-centered environmentNursing Care FunctionsProvide direct nursing care, including medication administration and treatmentsMonitor residents for changes in condition and notify physicians and families as neededAdmit, transfer, and discharge residents as requiredReview nursing documentation for accuracy and completenessEnsure infection control, safety, and emergency procedures are followedAssist with care planning, MDS coordination, and discharge planningSafety, Infection Control & EquipmentEnsure compliance with infection control and safety standardsMonitor use and maintenance of nursing equipment and suppliesReport hazardous conditions, equipment issues, and safety concerns promptlySupervisory ResponsibilitiesAssist with the overall supervision and management of nursing staff during the assigned shiftDelegated authority, responsibility, and accountability for assigned dutiesQualificationsEducation & ExperienceGraduate of an accredited nursing programMinimum 2 years of nursing experience, preferably in a skilled nursing or long-term care settingTraining in rehabilitative and restorative nursing practices preferredLicensure & CertificationsCurrent, active, unencumbered California RN licenseCurrent CPR certificationKnowledge and experience with PointClickCare (PCC) preferredMust maintain all required continuing education and licensure in good standingSkills & AbilitiesStrong leadership, clinical judgment, and decision-making skillsAbility to work independently during overnight hoursExcellent communication and organizational skillsCompassionate, patient-focused, and team-orientedPhysical DemandsAbility to stand, walk, lift, push, and pull up to 25 poundsAbility to respond to emergencies, including resident evacuation if necessaryAbility to cope with physical, mental, and emotional demands of the roleReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsWork EnvironmentSkilled nursing and long-term care environmentFrequent interaction with residents, staff, physicians, and familiesExposure to healthcare-related risks including infectious diseasesNOC shift may include weekends, holidays, and on-call situations as neededWe are an equal opportunity employer.
