RN - MDS Director

STGi is currently seeking a Minimum Data Set Director (MDS) to provide services at the Georgia War Veterans Home. The primary purpose of this position is to oversee the MDS process, ensuring compliance with state and federal regulations, and promoting proper clinical reimbursement for services rendered. The role involves chart assessments and collaboration with the healthcare team to create applicable healthcare plans for current and incoming residents.

Essential Functions

• Complete and guarantee the accuracy of the MDS process for all residents.
• Oversee collection and analysis of patient data to ensure accurate assessments and documentation.
• Work with nurses, doctors, and other healthcare professionals to develop individualized care plans for residents.
• Adhere to all state and federal documentation requirements, participating in regular audits and inspections.
• Develop and implement plans of action to correct identified quality deficiencies.
• Complete necessary forms and reports, submitting them to the Administrator as required.
• Host and attend interdisciplinary meetings to discuss resident needs and care plans.
• Explain the level of care provided and address any concerns or questions.
• Check the facility’s census daily and complete discharge and re‑entry tracking forms as needed.
• Track and schedule required resident assessments per state and federal requirements.
• Inform care team members of assessment scheduling changes and status updates.
• Review daily 24‑hour reports and telephone orders for significant changes that might alter assessments and care plans.
• Participate in facility surveys as necessary or directed.
• Complete resident assessments to capture the highest possible Patient Driven Payment Model (PDPM) score.
• Educate and train staff about the assessment process.
• Work as an RN, rounding with physicians as needed for optimal staffing.
• Complete all Minimum Data Sets (MDS) assessments and Care Area Assessment Summaries (CAAs).
• Participate in facility quality of care committees and meetings.
• Ensure Unit Nurses complete special care quarterly assessments.

Required Skills

• Long‑term care experience.
• Prior clinical healthcare experience, including direct experience with MDS.
• Ability to make independent decisions timely when circumstances warrant.
• Supportive team player, contributing to and exemplifying teamwork.
• Ability to interact tactfully with personnel, residents, families, visitors, government agencies, and the public.
• Trustworthy, reliable, and holding themselves to high ethical standards.
• Leadership and supervisory ability, willingness to work harmoniously with other personnel.
• Respect for the nursing profession, maintaining accountability, integrity, and quality patient care.

Required Education

• Minimum of a two‑year degree.
• Registered Nurse.
• Preferred certifications: RAI, RAC, CRC, or CMAC.

Working Conditions / Physical Requirements

• Work in office areas as well as throughout the facility.
• Subject to frequent interruptions.
• Interact with residents, family members, personnel, and visitors under all conditions.
• Work beyond normal hours, including weekends and holidays when necessary.
