You will manage the MDS workflow, ensure accurate documentation, and collaborate with Nursing, Therapy, Social Services, Dietary, and Activities teams to develop person-centered care plans.
Ideal candidates will hold an active RN license, have experience in skilled nursing, and be proficient with MDS tools such as SimpleLTC and PCC.
