The RN provides comprehensive, resident-centered care using the nursing process, focusing on holistic health needs, advocacy, and coordination of care across the continuum.
Responsibilities
- Perform ongoing assessments, develop individualized care plans, and monitor resident conditions for changes
- Administer medications, perform clinical procedures, and manage urgent or emergent care situations
- Coordinate care with interdisciplinary teams while educating residents and families on treatment and discharge plans
Requirements
- License: RN MT or Compact
- Experience: Rehab/LTC/Nursing Home RN – 1 year recent/current
