RN Integrated Care Manager — Community-Based Health Navigator

CareSource is hiring an Integrated Care Clinical Manager in Massachusetts. The role is a community-based nursing position focusing on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs.
