The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. In 2020, our program distinguished itself as thethird in the nationto achieveHeart Failure Certification through DNV, reflecting commitment to evidence-based standards and a culture of continuous improvement. Our multidisciplinary team plays a critical role in advancing patient outcomes through comprehensive education, individualized transition planning, and long-term disease management. The heart failure nurse is central to this work, providing expert nursing care byutilizingthe nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum.
