Job Summary:
The Primary Registered Nurse (RN) in Community Hospice provides compassionate, skilled nursing care to patients facing serious illness, focusing on symptom management and personalized care. The RN leads an interdisciplinary team, manages an assigned caseload, and supports patients and caregivers in various home settings including private residences, nursing homes, and assisted living facilities. This role emphasizes strong clinical skills, teaching, assessment, and critical thinking to improve patient quality of life.
Responsibilities: Required Skills & Certifications:
- Active Registered Nurse license in the state of practice.
- Minimum of 2 years nursing experience, preferably in hospice, home health, or related fields.
- Strong clinical assessment, symptom management, and critical thinking skills.
- Excellent communication and interpersonal skills.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and reliable automobile for fieldwork.
Preferred Skills & Certifications:
- Experience in hospice or palliative care nursing.
- Proficiency in interdisciplinary team collaboration.
Special Considerations:
- Work involves travel to patient homes and various care facilities.
- Occasional weekend back-up and on-call duties required.
- Work environment includes private homes, nursing homes, group homes, and assisted living facilities.
Scheduling:
- Weekday schedule: Monday through Friday, 8:00 AM to 5:00 PM.
- Occasional weekend back-up shifts and on-call responsibilities.
