RN - Hospice Case Manager

Take charge of developing, implementing, and managing personalized patient care plans in a hospice and homecare setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes. Deliver exceptional customer service while fostering respectful and supportive relationships with patients, families, and colleagues.

Essential Functions
  • Create individualized care plans based on medical and nursing diagnoses for patients ranging from newborns to the elderly.
  • Provide skilled nursing services aligned with the established care plan.
  • Complete accurate OASIS assessments as required.
  • Coordinate with physicians, patients, families, and other care providers to update and revise care plans.
  • Supervise Home Health Aide services according to policy guidelines.
  • Participate in on-call rotations to ensure 24/7 nursing availability for hospice, homecare, and private duty patients, including scheduled visits and admissions.
  • Maintain timely and precise documentation following agency policies.
  • Build and sustain positive working relationships with patients, families, healthcare professionals, and referral sources.
  • Encourage patient independence by setting care goals and involving patients and families in discussions about medical issues, testing, medications, and self-care skills.
  • Observe patients' physical, mental, emotional, and spiritual conditions and collaborate with team members as appropriate.
  • Adhere strictly to HIPAA regulations regarding Protected Health Information (PHI).
Minimum Knowledge and Skills
  • Proficiency with MS Office and keyboarding skills.
  • Strong communication abilities.
  • Effective multitasking and prioritization.
  • Basic nursing principles application.

Education and Experience
  • Graduate of an accredited nursing program.
  • Several years of clinical nursing experience.
  • BSN preferred.
  • Previous hospice and/or home health experience preferred.
  • Certification as a Hospice and Palliative Care Nurse preferred.
Licenses and Certifications
  • Current Registered Nurse license in the relevant state.
  • BLS certification required within three months of hire.
  • Valid driver's license and auto insurance meeting state requirements.
