Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to provide skilled home health visits to patients in their homes. This position begins as part-time and is ideal for an RN who enjoys independent work, flexible scheduling, and one-on-one patient care. Driving to patient homes is required.
Responsibilities
- Provide skilled nursing care in patients' homes per physician orders and care plans
- Perform comprehensive assessments, vital signs, and skilled nursing interventions
- Administer medications, wound care, injections, and treatments
- Educate patients and caregivers on medications, disease processes, and safety
- Monitor patient progress and report changes in condition
- Complete accurate, timely documentation for each visit
- Communicate with physicians, case managers, and interdisciplinary team members
- Follow all home health regulations and agency policies
Qualifications
- Active Massachusetts Registered Nurse (RN) license in good standing
- Minimum 1 year of RN experience (home health experience preferred, not required)
- Strong assessment, documentation, and communication skills
- Ability to work independently and manage time efficiently
- Valid driver's license, reliable transportation, and willingness to travel
- Comfortable using EMR and mobile documentation systems
Schedule
- Per-diem, Part-time to start
