Job Description
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on factors influencing patient/family health status and incorporate into the POC.
- Maintain accurate and up-to-date patient records, documenting all visits, contacts, problems, plans, and goals.
- Provide care using infection control measures to protect staff and patients.
- Educate patients and families on care, medication, diet, and safety, utilizing appropriate teaching methods.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality service.
Critical Information
- Schedule: 30 units on call required
- Remote/hybrid work based on schedule
- On-call responsibilities included
- Environment involves home care settings and compliance with health and safety standards
Education/Licenses Needed
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
- Bachelor of Science in Nursing (preferred)
- Current state license as a Registered Nurse
- Proof of current CPR certification
- CHPN certification (preferred)
