Responsibilities:

• Provide nursing care to patients in a home setting, following physician orders and care plans
• Administer medications and treatments as prescribed
• Monitor patient progress and report findings to the healthcare team
• Educate patients and families on health management and safety
• Document patient care activities accurately using Homecare Homebase EMR
