Job Summary

Home Health RN – Provide high‑quality, personalized nursing care in patients’ homes.

Responsibilities

• Lead clinical care by delivering high‑quality, personalized nursing services in the patient’s home environment.
• Develop, implement, and adjust individualized care plans that promote patient independence and well‑being.
• Collaborate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic, coordinated care.
• Provide education and emotional support to patients and families.
• Mentor and guide a care team that may include LPNs, LVNs, and Home Health aides, fostering a cohesive team environment.
• Utilize technology and best practices to optimize care delivery and maintain accurate documentation.

Qualifications

• Graduate of an accredited nursing program and holds an active RN license in the state served.
• Minimum of one year of nursing experience (home‑health experience a significant plus).
• Demonstrated critical thinking, strong clinical expertise, and case‑management skills.
• Strong commitment to patient‑centered care.
• Current CPR/BLS certification.
• Valid driver’s license with reliable transportation.

Compensation

Pay Range: $35.53 – $52.85 per hour
