As a Home Health RN, You Will

• Be a clinical leader, delivering high-quality, personalized nursing care in patients’ homes.
• Craft individualized care plans that promote patient independence and well‑being.
• Collaborate and coordinate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
• Empower and educate patients and families, providing crucial teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides, fostering a strong, cohesive team.
• Innovate with technology, utilizing leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience; Home Health experience required.
• Exceptional critical thinking, proven clinical expertise, strong case‑management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Pay Range

$36.51 - $59.00 /hour
