Schedule: Monday-Friday days with on-call rotation
Job Type: 13-week contract
Responsibilities:
Provide skilled nursing care in patients’ homes, conduct assessments, develop care plans, monitor progress, educate patients/families, coordinate with care teams, and maintain accurate EMR documentation.
Requirements:
Active Colorado RN license or compact license, BLS/BCLS certification, valid driver’s license, reliable transportation, and strong communication and critical-thinking skills.
Preferred:
Home health experience and familiarity with Homecare Homebase (HCHB).
