Position Summary
The RN Case Manager is responsible for overseeing a caseload of home‑health patients, completing skilled nursing visits, developing individualized care plans, coordinating interdisciplinary services, and ensuring compliance with state and federal regulations. This role is ideal for a nurse who thrives in a flexible, autonomous environment and enjoys building meaningful relationships with clients and families.
Key Responsibilities
- Complete initial and ongoing in‑home assessments , identifying physical, psychosocial, and environmental needs.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.
- Provide skilled nursing services , including wound care, medication management, disease‑specific interventions, and patient education.
- Communicate changes in condition to physicians and obtain updated orders as needed.
- Document all care accurately and timely in the EMR, including OASIS assessments when applicable.
- Participate in interdisciplinary case conferences to ensure coordinated, high‑quality care.
- Educate patients and caregivers on disease processes, safety, medications, and self‑management strategies.
- Support safe discharge planning and continuity of care.
Qualifications
- Active Registered Nurse (RN) license in the state of Illinois
- Minimum 1 year of nursing experience; home‑health experience preferred.
- Strong clinical judgment and critical‑thinking skills.
- Excellent communication, organization, and documentation abilities.
- Valid driver's license and reliable transportation.
