RN Home Health Case Manager 90% Saturday Rotation

Job Details: Location: Field Staff – Chico, CA 95973. Salary Range: $50.00 – $60.00 hourly. Internal Opportunity – 90% Position.

We’re excited to announce an internal opportunity for a 90% role for team members who are looking to take on a larger, consistent weekly workload while continuing to support excellent patient care. Your hourly wage will not be adjusted but your schedule will!

Schedule:
Week 1: SAT ON, SUN OFF, MON OFF, TUE ON, WED ON, THU ON, FRI ON
Week 2: SAT OFF, SUN OFF, MON OFF, TUE ON, WED ON, THU ON, FRI ON
Week 3: SAT ON, SUN OFF, MON OFF, TUE ON, WED ON, THU ON, FRI ON
Week 4: SAT OFF, SUN OFF, MON OFF, TUE ON, WED ON, THU ON, FRI ON

Qualifications

• Strong attendance and reliability – You follow your schedule, communicate proactively, and maintain dependable availability.
• Independent thinking and problem-solving – You navigate day‑to‑day decisions confidently and manage your workflow with minimal oversight.
• Timely documentation & queue management – Documentation is completed within standards, and your queue stays current and well‑managed.
• Excellent communication skills – You communicate clearly, professionally, and proactively with patients, families, and your team.

Weekend Requirement

This 90% role includes participation in the weekend day rotation. Weekend rotation days are considered blackout days. Requesting them off must be discussed in advance, and approval is not guaranteed.
