As an RN Home Case Manager, you will provide high-quality nursing care to patients in their homes, focusing on assessments, complex nursing treatments, and education to prevent readmissions. This role is ideal for experienced nurses who thrive in a supportive environment and enjoy working independently.

• Pay: $73,840 - $115,648/year

• Schedule: Full-time, Day shift

What You'll Do

• Provide direct nursing care to patients in their homes.
• Perform accurate and complete admission assessments and reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Serve as a primary coordinator of all disciplines for patient care.
• Assess, document, prioritize, and manage patients’ pain.
• Complete documentation and make calls after patient visits.
• Attend meetings and coordinate patient care.
• Arrange visit schedules for patients.

Requirements

Education

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience

• One year of current experience as an RN
• One year of experience in an acute Medical/Surgical or Home Care setting

Licenses & Certifications

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Valid Ohio Driver's License upon hire
• Proof of $100,000/$300,000 automobile insurance coverage upon hire
